Dorothy Yeung, Researcher of Our Hong Kong Foundation (OHKF), stated that Hong Kong’s current end-of-life care system still experiences gaps in service continuity, particularly during the transition from hospital to community care. The report recommends establishing a digital platform to facilitate collaboration between healthcare and social welfare services, by connecting healthcare professionals, social service providers, patients, and carers, to promote timely information sharing and improve service coordination.
